1. Click HERE for:
- Parent/Guardian Permission Notification Form- allowing their daughter to go to camp.
- Transportation Exemption Form- since we are not taking a bus to camp, this form is acknowledging how their daughter is getting to and from camp.
- Authorization for use of privately owned vehicle- FOR THOSE ACTUALLY DRIVING- must provide the needed info listed on the form.
- USS Summer Camp Release Form.
